Job Summary
This position provides professional services work in the area of intensive case management for high-risk mental health clients within Riverview Psychiatric Center and during their transition into the community.
As an Intensive Case Manager, you will act as a single identifiable support in a comprehensive and individualized approach to transition clients from the hospital into the community ensuring basic living needs are met and assisting clients in gaining access to medical and psychiatric requirements, rehabilitation, social, peer, educational, psychological support services.
