Job Summary
The primary purpose of this position is to oversee the facility to ensure all resident assessments are completed per strict guidelines.
This role requires accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change evaluations.
The incumbent must coordinate interdisciplinary participation in completing the Minimum Data Set for each new admission according to regulatory time frames.

PDPM tool utilization tracking
Interdisciplinary care plan coordination
Federal and state regulatory compliance
Level of care determination and notification
Nice-to-have
Experience with PCC software systems
Microsoft Suite proficiency
Long-term care facility experience
Data analysis for QM and QASP performance
Training new MDS coordinators
Key Requirements
Bachelor of Science Degree in Nursing preferred
3 years of supervisory experience preferred
Must be in good standing with State Board of Nursing
Maintain required continuing education and licensing requirements
