The County of Galveston is seeking a Case Manager for its Mental Health Court Program to assess participant needs and connect them with appropriate services. This role requires a Bachelor’s Level Clinician with at least two years of case management experience, strong communication skills, and the ability to work collaboratively within a team

Job Summary

  • The Case Manager assesses the nature and degree of GCMHC participant service needs and develops, implements and monitors the efficacy of the Court Re-Entry & Treatment Plan.
  • The Case Manager identifies current and emerging participant needs and links participants with appropriate rehabilitative services including mental health and substance abuse treatment, housing, food, transportation and other community resources.
  • The Case Manager will monitor participant’s compliance with the treatment plan by contacting treatment providers and assisting participants with non-compliance, through peer counseling and affirmations.
