  • Master's degree in Physical Therapy (graduated prior to 2016)
  • Doctorate degree in Physical Therapy
  • Licensed Physical Therapist (Arkansas)
  • 2 years of experience in Physical Therapy
  • Proof of legal authority to work in the United States
