The Case Manager will play a pivotal role in the success of the Pathway Home 6 (PH6) reentry initiative, supporting job seekers pre- and post-release to develop Individualized Development Plans (IDPs), overcome reentry barriers, and gain employment.
Responsibilities include managing participant documentation, facilitating intake and assessments, providing direct services, coordinating training and employment opportunities, and connecting job seekers to supportive services.
The role requires strong communication, organizational, and interpersonal skills to collaborate with various stakeholders while maintaining accurate documentation and compliance with federal performance standards.
