The primary purpose of this position is to provide each assigned resident with routine daily care and services in accordance with their assessment and care plan.
Employees must report all changes in resident condition, accidents, incidents, and suspected violations of privacy to the Health and Wellness Director or Executive Director immediately.
The role requires assisting residents with personal functions such as bathing, dressing, feeding, and toileting while ensuring their safety and comfort throughout the shift.

Must-have
Assist residents with daily living activities
Monitor and report changes in resident condition
Maintain confidentiality of protected health information
Perform lifting and transferring of residents
Document care entries in PCC system

Key Requirements
High school education or equivalent required
Proficiency in Microsoft Suite products required
