Job Summary

  • The primary purpose of this position is to oversee the facility to ensure all resident assessments are completed per strict guidelines.
  • This role involves tracking and monitoring the effective utilization of PDPM tools including Physician Diagnosis Verification and Weekly CSR reports.
  • The incumbent must coordinate interdisciplinary participation in completing the Minimum Data Set for each new admission according to regulatory time frames.

Key Requirements

  • Bachelor of Science Degree in Nursing preferred
  • Must be in good standing with State Board of Nursing
  • 3 years of supervisory experience preferred
  • Long Term Care Experience Preferred
  • Proficient in Microsoft Suite products
