Job Summary

  • This role offers the best of both worlds by combining the autonomy of community-based nursing with the clinical challenge of caring for patients with acute, inpatient-level needs.
  • You will provide high-impact, patient-centered care by completing thorough nursing assessments and creating individualized care plans directly in the patient's home.
  • The position supports a diverse adult patient population including those with heart failure, COPD exacerbations, and infections requiring IV therapies.
