Job Summary
Provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence.
Establish eligibility for services, obtain client histories, perform initial assessments, develop a plan to provide basic services, explain available services and programs, and refer clients to appropriate programs/agencies for other needed services.

Key Requirements
Graduation from accredited School of Nursing
RN certification with one year experience preferred
