Job Summary

  • The primary purpose of your job position is to oversee the facility to ensure assessments are completed per guidelines with a focus on the resident assessment process including individual assessment, MDS assessment, and resident plan of care.
  • You will provide oversight on facilities’ weekly PDPM meetings, track and monitor performance metrics, and provide data analysis and clinical recommendations for optimum outcomes.
  • This role involves coordinating interdisciplinary participation, ensuring compliance with federal and state regulations, maintaining confidentiality of resident care information, and conducting resident care conferences.

Key Requirements

  • Graduation from Accredited School of Nursing
  • Bachelor of Science Degree in Nursing preferred
  • Good standing with State Board of Nursing
  • Maintain continuing education/licensing
  • 3 years supervisory experience preferred
