Job Summary

  • Provides direct patient care to hospice patients in a variety of settings and levels of care under the direct supervision of an RN.
  • Responsibilities include providing care to the terminally ill patient, reinforcing patient and family training, performing patient assessment and documentation, and participating in the comprehensive Interdisciplinary Plan of Care.
  • All services will be provided 24 hours a day, 7 days a week as assigned to a variety of teams and duties, including nights and weekends, with on-call rotations as assigned.

  • Valid Driver's license with a good driving record
  • Personal car in good working order
