This role provides critical oversight on weekly PDPM meetings and monitors facility utilization of PDPM tools to ensure optimum outcomes.
The incumbent is responsible for assessing health status, determining levels of care, and ensuring timely completion of all MDS assessments including PPS Medicare and significant changes.
The position requires coordinating interdisciplinary participation in completing the Minimum Data Set according to strict regulatory time frames while maintaining confidentiality of resident information.
