Job Summary
The Medical Home Nurse Care Manager facilitates coordination of care and communication, supporting patients with complex medical, behavioral and/or social issues.
Responsibilities include identifying and enrolling eligible patients, assessing health status, developing care plans, and providing hospital discharge follow up.
The role requires applying principles of purposeful, organized thinking to make sound clinical judgment/decisions and documenting outcomes as patients progress.

Key Requirements
Graduate from accredited Nursing program
BSN required
New Hampshire Board of Nursing license
