Job Summary
This role provides critical oversight on weekly PDPM meetings and ensures effective utilization of PDPM tools for skilled patients.
The incumbent is responsible for assessing new admissions, determining levels of care, and coordinating interdisciplinary participation in completing Minimum Data Set assessments within regulatory timeframes.
The position requires maintaining accurate schedules for all MDS assessments and generating appropriate forms for level acuity changes as required by state law.
