Habitat Health is seeking a Personal Care Attendant on a per diem basis in Sacramento, CA, to support daily living and safety needs for older adults in their PACE center and homes. Job Summary

  • Habitat Health provides personalized, coordinated clinical and social care as well as health plan coverage through the Program of All-Inclusive Care for the Elderly (“PACE”).
  • This role involves attending to the daily living and safety needs of participants in our PACE center and occasionally in the home.
  • Responsibilities include assisting participants with personal hygiene, activities of daily living, and fulfilling their requests to ensure the best possible experience.
