Provides medically related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.
Assists residents with health care decisions, personal shopping, and securing appropriate prosthetics and assistive devices, while coordinating with interdisciplinary teams and community agencies.
