Job Summary

  • You will work as a Care Coordinator to provide holistic care coordination to patients with complex needs to improve their health outcomes and prevent avoidable hospital presentations.
  • Provide targeted dietary assessment, intervention, lifestyle coaching and clinical management of dietary related conditions and health behaviours.
  • This position is part of the Healthcare for the Homeless program (HfH) and has strong links with the Health Independence Program (HIP).
