  • Doctor of Medicine (MD) degree required
  • Completion of ACGME accredited residency in Otolaryngology
  • Board certified or board eligible by ABOHNS
  • Eligible for medical licensure in the State of Ohio
  • Fellowship in Laryngology from American Laryngological Association
