Job Summary

  • This on-site role serves as a key liaison within a partnered dialysis facility, ensuring coordinated, patient-centered care through structured coordination, strong clinical collaboration, and proactive communication.
  • The Care Coordinator works directly within one of our partnered-dialysis clinics to engage patients in person, coordinate care plans in collaboration with Ennoble providers and facility staff, schedule and organize provider visits, and ensure timely follow-up and clear communication with patients, families, and the clinical team.

Key Requirements

  • Active, unrestricted clinical certification or licensure
  • Dialysis-specific certification preferred
  • Experience managing provider schedules preferred
  • Minimum one (1) year of experience with hemodialysis/nephrology patients
  • Proficiency in Microsoft 365
