Job Summary

  • Become a part of our caring community and help us put health first Humana Gold Plus Integrated is seeking Long-Term Services and Support (LTSS) Care Coordinators (Care Coach 1) in the state of Illinois to assess and evaluate members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families towards and facilitate interaction with resources appropriate for the care and wellbeing of members.
  • Visits Medicaid members in their homes, Assisted Living Facilities (ALFs), and/or Long-Term Care Facilities and other care settings – 75-90% local travel.

Key Requirements

  • Bachelor's degree in social sciences
  • Licensed Practical Nurse (LPN) in Illinois
  • 1 year experience with elderly individuals
  • 1+ years healthcare and case management experience
  • 1+ years Medicare and Medicaid recipients experience
  • Intermediate to advanced computer skills
  • Valid state driver's license
  • Personal vehicle liability insurance
