This role involves tracking and monitoring facility effective utilization of PDPM tools including Physician Diagnosis Verification and Admission/Discharge CSR.
The incumbent must assess and determine the health status and level of care of all new admissions while ensuring timely documentation.

Skills & Requirements
Must-have
Oversee weekly PDPM meetings for skilled patients
Ensure accurate MDS assessment completion
Coordinate interdisciplinary resident care conferences
Monitor QM and QASP performance metrics
Maintain federal and state regulatory compliance
Nice-to-have
Experience with PCC software systems
Proficiency in Microsoft Suite products
Long term care facility experience preferred
Ability to provide clinical recommendations
Key Requirements
Bachelor of Science Degree in Nursing preferred
Must be in good standing with State Board of Nursing
