Key Requirements
State license to practice Medical Doctor in AZ with full prescribing authority
Completion of Medical School, Family Medicine Residency, and Internal Medicine fellowship
Board certified in Family Medicine
Board certified or board eligible in Internal Medicine with certification within one year
Valid Fingerprint Clearance Card or ability to obtain
Reliable transportation, valid driver's license, and proof of auto insurance
Basic Life Support certification or ability to be trained within 10 business days
TB Screening Record and PPD/QuantiFERON/Chest x-ray upon hire
Required vaccinations or declination forms: Flu, Hep B, MMR, Tdap, Varicella
