Job Summary
Assist in planning, developing, organizing, implementing, evaluating, and directing facility's social service programs to meet resident's emotional and social needs.
Provide consultation to staff and community agencies to solve resident needs and problems through social service programs.
Assist in discharge planning with appropriate agencies and coordinate social service activities with other departments.
