The PACE Driver is responsible for providing safe, timely transportation to frail elderly participants while communicating relevant plan of care information with the interdisciplinary team.
This role requires direct interaction with caregivers and partners to set the tone for a joyful care experience that supports seniors living in their own homes.
Drivers must be proficient in using medical equipment such as power wheelchairs and oxygen, securing all items safely during transit, and documenting encounters in the Electronic Medical Record.
