Job Summary

• The primary purpose of this job position is to assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
• Essential duties include meeting with administration and staff in planning social services, developing and implementing policies for identifying resident needs, and providing consultation to staff and community agencies.
• The role involves working with emotional problems, assisting residents and families with anxieties, and coordinating social service activities with other departments and community agencies.
