Coordinates discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.

Key Requirements
Minimum of two (2) years utilization review/case management experience
Minimum of two (2) years social work experience
Maintain current professional licensure in nursing

Bachelors or Masters Degree in related healthcare field may be considered
