This role provides oversight on facilities' weekly PDPM meetings and tracks the effective utilization of PDPM tools for skilled patients.
The incumbent is responsible for assessing new admissions, ensuring accurate MDS completion, and coordinating interdisciplinary participation in care planning.
Candidates must maintain confidentiality of resident care information while supervising staff and managing level of care changes within the facility.

3 years supervisory experience
Long Term Care experience
PCC software knowledge
Microsoft Suite proficiency
