Job Summary

  • Acts as a patient advocate/case manager to hospital clients, coordinating, negotiating, and procuring services and resources for complex patients to facilitate discharge and achieve quality and cost-efficient outcomes.
  • Provides supervision and acts as a resource to unit-based care management team members, assisting in the identification of appropriate utilization of resources across the continuum of care.
  • Takes an active role in maintaining a high level of functioning in Multi- Disciplinary Rounds, performing audits, and reporting findings to the Director of Case Management.
