Job Summary

  • This job welcomes and connects the consumer to the health system, handling acquisition, front door navigation, care coordination and transitions of care.
  • The role involves taking incoming contacts via phone, chat, email, or text, determining the correct pathway for patients, and coordinating with clinic partners to resolve access barriers.
  • Develop and foster interpersonal relationships with stakeholders by offering patient-focused solutions and demonstrating an elevated level of attention, care, and follow-up to close the customer service loop.
