The primary purpose of this position is to provide each assigned resident with routine daily care and services in accordance with the resident's assessment and care plan.
Employees must report all changes in residents' conditions, accidents, incidents, and suspected violations of protected health information to supervisors immediately.
The role requires assisting residents with personal care functions including bathing, dressing, grooming, feeding, and mobility while maintaining a calm and warm environment.

Skills & Requirements
Must-have
Assist residents with daily living activities
Monitor and report changes in resident condition
Maintain confidentiality of protected health information
Perform lifting and moving of residents up to 25 pounds
Record entries on notes and incident reports
Follow established policies for blood/body fluid exposure

Create atmosphere of warmth and positive emphasis
Cooperate with inter-departmental personnel
Participate in regularly scheduled nursing meetings
Demonstrate ability to solve practical problems
Ability to effectively present information to managers
Key Requirements
High school education or equivalent
