Job Summary
This role is responsible for assisting in the planning, developing, and directing of social service programs to meet the medically related emotional and social needs of residents.
The position requires coordinating social service activities with other departments and assisting residents in achieving the highest practicable level of self-care and independence.
Employees must maintain confidentiality of all resident care information including protected health information while reporting any suspected incidents of fraud or unauthorized disclosure.

Key Requirements

One year supervised social work experience in healthcare

Completion of mandatory in-service trainings including HIPAA
Prior long-term care or skilled nursing facility experience preferred
