Skills & Requirements
Must-have
High school diploma or equivalent
Knowledge of Medicare/Medicaid regulations
Experience with resident discharge planning
Ability to maintain confidential health records
Strong written and verbal communication skills
Nice-to-have
Bachelor's degree in Social Work or related field
Prior healthcare facility experience preferred
Ability to assist with emotional support and counseling
