Job Summary

  • Drive partnership and engagement efforts with internal stakeholders and external partners to gain awareness of ongoing developments within the landscape, facilitate collaborations and share best practices.
  • Support dissemination and implementation of organizational change initiatives at the Cluster level and facilitate smooth change management.
  • Work collaboratively with key stakeholders such as the Regional Health Systems (RHS), Communities of Care (CoC) and Active Ageing Centres (AACs) to co-create a local community support network for seniors.
