Job Summary

  • The Service Navigator will develop connections between GP's, LACS, program managers and other services, to ensure they have a clear understanding of the psychosocial support programs and opportunities available in the community and within Wellways.
  • The Service Navigator will work in conjunction with participants to address key areas that support decision making, social skills, independent living skills, community access, healthy lifestyle, self-identity, self-esteem, and resilience.
