Job Summary

• Completes psychosocial assessments for patients' post-hospital care and assists them in coping with stress related to hospitalization, disability, or chronic/terminal illness.
• Works with the Clinical Care Management team to assess and evaluate patients for appropriate care levels and serves as a resource to hospital staff and physicians regarding emotional and social components of illness.
• Provides information and education to patients on community resources and options for post-hospital care, making referrals as needed, and reports suspected cases of abuse/neglect/exploitation.
