The role is responsible for ensuring patients referred from Summit Health locations are scheduled for appropriate follow-up care as part of a commitment to coordinated care.
The manager will lead a team of Supervisors and agents, optimizing workflows, managing time and attendance, and providing mentorship while collaborating with supported markets.
