Job Summary
Ensures the provision of quality patient care in the appropriate setting through care coordination, case management, utilization management of inpatient admissions, and transitions of care to different levels of care.
Collaborates with the attending practitioner, the family and other members of the health care team, coordinates individualized plans of care, and communicates the plan to appropriate stakeholders.
Facilitates continuity of care by coordinating transition to post-discharge care and practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses.
