Job Summary

  • The Medical Social Worker provides patient/family psychosocial consultation, interventions and resources which may include, but not limited to, Social Drivers of Health (SDOH), family and community support systems, substance use, and protective factors.
  • The Social Worker works closely with the RN Case Manager/Transition Planner and interdisciplinary team to ensure safe and timely discharge.
