  • The Senior Community Population Health Coordinator will oversee the development and execution of programming and services centered on prevention and management of chronic disease, and addressing social drivers of health.
  • This position will create structured pathways that enable patients to receive evidence-based lifestyle interventions, while simultaneously providing OSU students with hands-on training in community health settings.
  • The Senior Population Health Coordinator leads population‑level initiatives to improve health outcomes for individuals and families served by the Healthy Community Center in Columbus, Ohio.
