Salary
Base: $44.15 - $66.25 per hour; Bonus/Equity: $10,000 sign-on bonus paid in two installments; Benefits: Comprehensive benefits including health, retirement, PTO, and educational assistance

Key Requirements
Licensed Physical Therapist in Illinois or eligible
Minimum 1 year Physical Therapy clinical experience
Ability to lift up to 35 pounds unassisted
Proficient computer skills including scheduling and documentation systems
