Haywood Post Acute is seeking a full-time MDS Assistant (LVN) to oversee weekly PDPM meetings and ensure the accurate completion of MDS assessments for skilled patients. Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
  • Tracks and monitors QM and QASP performance monthly and quarterly, providing data analysis and clinical recommendations.
  • Ensures accurate and timely completion of all MDS Assessments, coordinates interdisciplinary participation, and assists in formulating and revising care plans.

  • 3 years of supervisory experience preferred
