Job Summary

• Plans and provides individual and group instruction to meet requirements established in the National Standards for Diabetes Self-Management Education Programs and American Diabetes Association for participants in the Diabetes Self-Management Program.
• Provides advanced and specialized knowledge and communicates recommendations for changes in patient treatment to physicians and hospital staff; documents recommendations and monitors changes and outcomes; facilitates the development and implementation policies, procedures and protocols related to diabetes management.

• 1-2 years related clinical experience
• Current RN licensure or RD Registration
