Job Summary
This role is responsible for creating, implementing, and evaluating care plans for patients across the health continuum while utilizing nationally recognized standards of practice.
Incumbents will serve as key team members in managing readmissions, length of stay, ED utilization, and patient satisfaction through effective collaboration with providers and external partners.
The position requires a Bachelor's Degree in Nursing or equivalent experience along with current Pennsylvania RN licensure to ensure safe and effective care coordination.
