Job Summary
The Registered Nurse will provide patients who have chronic conditions the support, the education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state.
Will work with the individual patient, their families, and providers of care to develop and implement individual health plans, and provide support to the patient in self-management.

Key Requirements
Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
Valid RN License
Minimum of 1 year Registered Nursing experience
Baccalaureate Degree, ADN, or Diploma RN Degree
3 years RN experience with 2 year experience in a clinical environment
Current BLS certification
ACLS certification or obtain within one year
Knowledge or experience in PCMH
Supervisory experience
