Job Summary
Function as a role model, mentor, teacher, and ally as you communicate hope and encouragement.
Assist patients during their inpatient LTCC hospitalization and the 90 days post discharge to overcome obstacles and identify their own health, wellness and recovery goals.
Partner with treatment team to facilitate a successful discharge and transition into the community.

Key Requirements
High school graduation or equivalent
One year of professional experience as a Peer Advocate/Peer Counselor
Peer Counselor Certification OR Community Health Worker Certification in WA State
