Job Summary

  • Function as a role model, mentor, teacher, and ally as you communicate hope and encouragement.
  • Assist patients during their inpatient LTCC hospitalization and the 90 days post discharge to overcome obstacles and identify their own health, wellness and recovery goals.
  • Partner with treatment team to facilitate a successful discharge and transition into the community.
