Cleveland Clinic Care Coordinators are successful in helping patients manage their own care, decreasing Emergency Department visits, observation status, inpatient stays, and hospital readmissions.

This is a hybrid position, working Monday to Friday, 8:00 AM – 5:00 PM, with one day a week remote from home.
