Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact on their medical condition, treatment, recovery and safe transition from one care environment to another.
Core competencies include psychosocial assessment, counseling and crisis intervention skills, knowledge of post acute levels of care and the PASAAR process for extended care facility placement, assessment and counseling processes involved, and skills assessing neglect/abuse, including substance abuse and domestic violence.
