Job Summary

  • Saint Alphonsus Medical Group is a physician led and accountable 650+ provider multispecialty group seeking a Palliative Medicine Advanced Practice Provider to join its inpatient and outpatient Palliative Care service.
  • Its team of Palliative providers provide hospital & outpatient, interdisciplinary team consultations for patients in need of Palliative Care and continuing education on Palliative Care topics for Saint Alphonsus health care providers.
  • The Palliative Medicine APP will work with members of interdisciplinary teams, collaborating daily with Palliative Medicine physicians, other Advanced Practice Providers, RNs, social workers, chaplains, pharmacists, rehabilitation therapists and other providers.
