The Primary Care Coordinator will manage 500-600 patients, serving as the central point of contact, coordinating care, maintaining facility relationships, and ensuring excellent healthcare delivery.

Serve as primary contact for patients

Key Requirements
High School Diploma (some college preferred)
Basic understanding of Primary Care Operations

Must be able to work ~5 on call overnights and/or weekends
Must not be on any exclusion or debarment from Federal Health Care Programs
