Job Summary

  • This position will collaborate with other professionals to evaluate patients' psychiatric condition and to assess client needs as part of the interdisciplinary team.
  • The per diem social worker works collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community, and assesses and arranges discharge planning needs and post hospital follow up disposition.

Key Requirements
